During a Robert Wood Johnson Foundation (RWJF) First Friday Google+ Hangout (11/7/14), panelists discussed building bridges between hospitals and health systems on the one hand, and public health and other community actors on the other, to improve population health. Susan Dentzer, senior policy adviser to the Foundation, led a discussion with experts sharing successful strategies and partnerships to address community health; the increased accountability hospitals now have to improve health through ‘community needs assessments’ and health improvement plans; and promising practices that give hospitals and health systems a role in tackling the factors that affect health outside of clinical settings.
